Information collected
The assessment request form collects your name, email, optional phone number, course and program interest, and any message you choose to submit.
Purpose and processing
Information is used to respond to your request and discuss availability, scheduling, and appropriate support. Form submissions are processed and stored through Netlify, the website hosting provider, and accessed by North Country Academic Coaching to handle inquiries.
Keep sensitive information out of the form
Please do not submit student records, diagnoses, medical details, financial account information, or passwords. This form does not authorize student text messaging; communication arrangements and parent permission are established separately during enrollment.
Questions or deletion requests
Use the contact form to request access, correction, or deletion of inquiry information. Records may need to be retained for applicable legal or business obligations.
